To request a Proximity Card which will be required for access to the Community Pool Facility and Restrooms after February 1, 2007, simply fill out and submit the request form below. This form must be complete with full name, complete address and all contact information. Once the form is complete, please click on the "Send" button at the bottom of the form.
The first card for each resident shall be provided and mailed free of charge to any resident who request one. Any additional cards or replacements for lost, damaged or stolen cards may be obtained for $15.00 each. Please indicate in the comment section below how many cards you are requesting and if these are additional or replacements.
Important: Payment for additional cards must be received in advance as cards will not be issued until full payment is received.
